How To Use Your Preventive Care

Through your preventive care service, you’ll get reimbursed for preventive medical services such as dental cleanings, vision exams, flu shots, and much more. Simply snap a picture of your bill and submit it for reimbursement right from your phone. Use your preventive care service anytime you visit a lab or doctor’s office for dozens of tests and services to keep you and your family healthy.


How do I get started? Your preventive care expense administrator is Group Dynamic. You can use their mobile app to snap a picture of your bill and submit it for reimbursement right from your phone. They also have a Web Application you can access from any computer.


To submit an expense for reimbursement, you will need to click here to activate your account with the following credentials:

First Name: [your first name]

Last Name: [your last name]

Zip Code: [your zip code]

Health Access ID: 000-00-0000
(Note: Enter your Health Access ID in place of Social Security Number.)

How do I get reimbursed?

  1. When you go in for a preventative screening or test, let your provider know that you are a “self-pay patient.” 

  2. Simply pay your provider directly and save the receipt. 

  3. Download and use Group Dynamic’s mobile app.

  4. Snap a picture of your bill and submit it for reimbursement right from your phone. Or upload the image on their web application to submit from any computer. 


Which services are eligible for 100% reimbursement? Dozens of preventive labs and screenings are available for 100% reimbursement up to $3,250 to keep you healthy, including dental and vision and mental health counseling.

  • Mental health treatment and counseling ($250 annually per member)

  • Smoking Cessation (one time $250 per member)

  • Dental cleaning and/or vision exam ($150 year 1 / $250 year 2+)

  • Well child exams

  • Physical exam (every calendar year)

  • Gynecological exam (every calendar year, age 21 and over)

  • Breast MRI and/or ultrasound (if required post-mamogram)

  • Procto/sigmoidoscopy (every 60 months)

  • Cologuard (every 36 months)

  • Flu and pneumonia shots (annually)

  • Lipid panel (cholesterol and triglycerides)

  • Comprehensive metabolic panel

  • Complete blood count with differential test

  • Thyroid panel TSH (thyroid stimulating hormone) reflex to T4 if abnormal

  • Routine comprehensive urinalysis

  • Hemoglobin A1C

  • Vitamin D level

  • Prostate specific antigen test, men age 50-75 (40 with family history)

  • Heart screening: c-reactive protein & homocysteine levels

  • Bone density screening (post-menopausal women, every 2-3 years)

 

Are there any network restrictions? No! There are no network restrictions for your preventive care benefit. You are free to visit any lab or doctor you’d like and you’ll be reimbursed up to $3,250 for the services above.
